The Challenge

In Greater Philadelphia and New Jersey, life expectancy can differ by many years between neighborhoods a short drive apart. The causes are well documented and largely sit outside the clinic: housing quality, food access, income, environmental exposure, chronic stress, and whether care is reachable, affordable, and trusted. The region has world-class medical institutions and persistent, patterned differences in who benefits from them.

Community-based organizations carry much of the work that actually moves these outcomes — outreach, navigation, prevention, maternal and infant health support, chronic disease management in the places people live. They do it with fragile funding, on grant cycles far shorter than the timelines health improvement requires, and usually without the evaluation infrastructure that would let them prove what they already know works. When a hospital or health plan looks for a community partner, the organizations with the deepest reach are often the hardest to see.
