A Community of Practice for the people building care that arrives before the crisis does, across Greater Philadelphia and New Jersey.
The Challenge
Demand for behavioral health support across Greater Philadelphia and New Jersey has outrun the region's capacity to meet it. Waits for care stretch past the point where the need is urgent. The workforce that delivers care is stretched, underpaid relative to the difficulty of the work, and leaving faster than it can be replaced. Young people are presenting with needs that schools and community organizations were never resourced to hold. And the overdose crisis continues to take lives in neighborhoods that have already absorbed decades of loss.
The system meant to respond is fragmented by design — split across health care, schools, housing, child welfare, and the justice system, each with separate funding, separate eligibility, and separate records. People in crisis experience that fragmentation as a series of doors that do not connect. The organizations working across those seams have learned an enormous amount about how to make the handoffs hold, and almost none of it is documented.
The Behavioral Health Hub exists to capture that cross-system knowledge and give it somewhere to go.
